To request to be a client please Email the following: 1. A picture of your insurance card (front and back). 2. A copy of your driver's license. 3. Your cell phone number. 4. Include the reasons you are seeking counseling services so we can identify if we can assist your needs as well as your counseling history. After the first appointment, Therapy Sessions cost $150 for 60min. All fees are due upon arrival prior to services.
Blue Cross and Blue Shield, United Health Care Caterpillar, Health Alliance, Group Administrators, Methodist First Choice & Consociate.
(Client is responsible for a $60 deposit each session until the insurance sends payment or EOB)
We do not accept All Kids, Medicaid, Meridian, Aetna, Cigna, OSF Health, Health Partners or Medicare.
