To request to be a client please 1. Email a picture of your insurance card (front and back). 2. Email a copy of your driver's license. 3. Email phone number. 4. If the appointment is for a child, email their full name and DOB. 5. Include the reasons you are seeking counseling services so we can identify if we can assist your needs. 6. After the first appointment, Therapy Sessions cost $150 for 60 Min or $200 for 90 Min. All fees are due upon arrival. If you cannot pay your appointment will be cancelled at you will be charged a $20 fee.
